Summer leave form (applies to the summer months July – August)

Sick leave form  (via the Department of Medicine Secretariat)

The service grants the sick leave upon request of the employee. The application for sick leave is submitted within seven (7) days of the employee's absence due to illness (Law 3528/2007 article 56)

 When it comes to short-term sick leave: up to 8 days per year (Law 4210/21-11-2013 article 2). These leaves are granted, either in full and all eight (08) days with the opinion of a private or public institution's treating physician, or six (06) days with the opinion of a private or public institution's treating physician and two (02) days with a responsible declaration by the employee (no but consecutive). Sick leave beyond eight (8) days per year is granted after the opinion of the relevant health committee. As an exception, the opinion of the relevant health committee is not required: a) when the leave is granted based on the opinion of the Clinical Director of a Public Hospital and if there has been previous hospitalization seven (07) days at least. b) when the license is granted after surgery in a public hospital or private clinic.
